SD Card Unreadable? On my DSLR and every computer I tested on?

So there's this SD card which I have in my possession right now which can't seem to open in every single computer that I tried on (it doesn't detect it AT ALL) and when plugged into my DSLR, the DSLR couldn't even turn on.

At first when I plugged it into a Macbook, I got it detected but there was a message saying that the card's format is unreadable. When the prompt came out it asked me to either "initialize", "ignore" or "eject". I clicked initialize and it brought me into a disk utility window. I closed the window, plugged out the SD card, and now it does not work again.

Any idea what's the problem here and if there is a fix for it?

Throw it away.

SD cards aren't the greatest medium ever developed, but they're here to stay, and if you can't format it in your camera, it's rubbish.

They're cheap enough that you can buy a couple of spares anyway.

You've answered your own question... it doesn't work any longer, replace it.

Anything on is is not readable now.

Put it in your camera and from the camera menu, Format the card. (Only ever format (initialize) a camera card in the camera it is to be used in.

If it still won't work then throw it away.
